What do you need to bring when applying for Medicare?
- Birth certificate.
- Proof of U.S. citizenship or legal residency.
- Your Social Security card (if already receiving SSA benefits)
- Health insurance information.
- Tax information.
- Military documents.
- Part B enrollment application.
Can I apply for Medicare through the mail?
You can apply online or you can mail your completed CMS 40B, Application for Enrollment in Medicare – Part B (Medical Insurance) to your local Social Security office. Note: If you don’t already have Part A you can apply online at
Does Medicare need bank statements?
Medicare plans and people who represent them can’t do any of these things: Ask for your Social Security Number, bank account number, or credit card information unless it’s needed to verify membership, determine enrollment eligibility, or process an enrollment request.
What do I get in the mail when I apply for Medicare?
If your application is approved, the Centers for Medicare & Medicaid Services will send you a Welcome to Medicare packet in the mail with your Medicare card. You will also receive the most current version of the Medicare & You handbook, with important information about your Medicare coverage choices.
Do you need to submit Social Security number when you sign up for Medicare?
If you sign up for Medicare online or by phone, you may need to submit additional information. Social Security will let you know if you need to do this and what that information might be.
Where do I Send my Medicare enrolment form?
Complete a Medicare enrolment form. Mail or email your form and supporting documents to Medicare Enrolment Services. If you include certified supporting documents, you’ll get a Medicare card in the mail. We’ll send it to the address you give us. You can also get a digital copy of your Medicare card.
When do you get Your Welcome to Medicare packet?
Medicare will send you a “Welcome to Medicare” packet 3 months before you turn 65. You’ll still have other important deadlines and actions to take, so read all of the materials in the packet. (If you live in Puerto Rico, you’ll only get Part A. If you want Part B, you need to sign up for it.)
